| By Texas Law, the
following immunizations are required prior to admittance for students in
Pre-K through fifth grade: Diphtheria/Pertussis/Tetanus (DPT) Polio (OPV) Measles/Mumps/Rubella (MMR) Haemophilus influenza type B (Hib) Hepatitus A Hepatitus B Varicella (chickenpox) The school nurse can provide information on age-appropriate doses or on an acceptable physician -validated history of illness required by the Texas Department of Health. Proof of immunization may be personal records from a licensed physician or public health clinic with a signature or rubber-stamp validation. If a student has had chickenpox, a statement from the parent, which includes the month and year of chickenpox disease, is required. |
If a student's religious beliefs or
conscience conflict with the
requirement that the student be immunized,
the parent must present an official
TDH affidavit form to the school within 30 days of enrollment. Only
the official TDH affidavit form is legally valid. Parents or guardians
need to request affidavit forms in writing. This statement must
be renewed yearly. For further information contact the school nurse.
If a student should not be immunized for medical reasons, the student or parent must present a certificate signed by a U.S. licensed physician stating that, in the doctor's opinion, the immunization required would be harmful to the health and well-being of the student or any member of the student's family or household. This certificate must be renewed yearly unless the physician specifies a life-long condition.
